Was it also ever explained why the amount of wishes Shenron could grant rose up to 2 in the Cell arc, but in the Boo arc it rose up to 3??

Black and Zamasu's fusion showed that it isn't a perfect, singular being. Same reason why Shin used the dragon balls to defuse, it didn't feel "right".
So a pregnant woman would return to how she was once fusion time depletes.
And SSJ2 & SSJ3 are mutations of SSJ, not the next step in the ladder of transformations. Vegeta didn't need SSJ3 to surpass Goku, none of them need SSJ2 & SSJ3 to surpass a SSJ3. Why Goku still uses it is a better question, I feel.
