If Cell would have broke out Kaio-Ken and still lost to ssj2 Gohan that would have been a huge increaae in power for Gohan.

I know Tapion mentioned already that he wasn't really 'controlling' Hirudegarn at this point, but maybe he's still retained enough to keep him from doing the smoke trick? I mean, just because Hirudegarn's body can do that doesn't mean Tapion's can. For all we know, Hirudegarn teleporting away like that would leave Tapion's body exposed even briefly, allowing Cell to go directly for him.

Its exactly the same. A lot of fights in Dragon Ball (and outside of Dragon Ball) have a fighter managing to dodge some attacks from their adversary but not all. This happens because their adversary is pretty powerful and fast and so, even though the fighters might be able to dodge some attacks, they will not be able to dodge all of the attacks. Turning to smoke might be even faster and more effective than any of those other options but it still relies on the power, attention, speed and reaction time of Hirudegarn like those other options do regarding other fighters.
Against an adversary powerful enough and fast enough to put Hirudegarn's power, attention, speed and reaction time in check, Hirudegarn will not be able to avoid everything, just like another fighter will not be able to dodge everything, even if he can dodge some of it.
Even in Hirudegarn's movie he wasn't able to dodge everything with his smoke ability. For example, Gotenks hits him with a complete barrage of Ki blasts. Why didn't he just avoid it with his smoke ability? For the same reasons a fighter might miss a dodge. Gotenks, his adversary, was powerful and fast enough to capitalize on the circumstances of the fight and so Hirudegarn was hit.
With Cell in this fight, it's similar. Even though Hirudegarn can dodge some of Cell's attacks with his smoke ability, Cell is strong and fast enough to be able to capitalize on the circumstances of the fight, to put Hirudegarn's power, attention, speed and reaction time in check, and hit him.
