The side story chapter, TRUNKS THE STORY -The Lone Warrior-
In the tankôbon release it's just stuck in there somewhere during the Cell arc (anyone know where, exactly?), and in the Kanzenban it's at the end of the last volume. But when was it published in Jump? And which chapter did it follow?

"TRUNKS THE STORY -The Lone Warrior-" appeared with Chapter 386 in the 1992 double issue of Weekly Jump, issues #36 and #37 (i.e. 1992 WJ #36/37). It was then released at the end of Tankoubon Volume #33, along with Chapters 386 to 396. Finally, as you said, it was released at the end of the last Kanzenban volume.
To give you a little bit of a timeline, this is how the releases were spread out:
August 30, 1992 => June 9, 1993 => April 2, 2004
Weekly Jump => Tankoubon => Kanzenban
