Piccolo and the Kaioken is actually one of my favorite "what if's" and " why didn't they do it" ideas. Here was a legitimate, non-asspullish method to keep him relevant as figther and help him keep up with SSJ level power ups. He was the only one who could handle the damage the Kaioken did to the body. Or maybe even nullify it. If his enhanced regeneration is something ki based, then the Kaioken might actually help him heal.Why didn't Goku ever teach the Kaioken to Gohan or Piccolo or anyone?

Kaioken was excluded from the plot after the Goku - Frieza fight. Simple as that.
