The middle Broly image is Restrained SSJ Broly, rather than False. It's kinda what Goku did in the post-Buu filler when he had to go help Pikkon deal with the villains in Hell acting up. Yeah, none of that was canon, but Goku basically used a mid-SSJ state to beat them up. His hair spiked up but didn't change color.

I believe Toriyama took the Restrained SSJ concept from the original movie and applied it differently. We can see that the collar is broken off, but Paragus may still have some other degree of control over Broly, hence why he hadn't gone full SSJ yet. Either that or Broly felt that Goku and co still weren't challenging enough to warrant him using his Full Power SSJ form.

SSG Vegeta doesn't make any sense in the anime. He never used it and now he got it?! Hope there's an explanation for that.
All the designs look great though.

I wasn't expecting Buu arc Vegeta. I'm glad to have that design back.

I think they want to keep things simple as it's easier to understand hair color changes and it's less work for the staff as they won't have to animate lightning for Ssj2 or long hair for 3.

I don't remember when specifically but a fan asked him in one of his posts if Vegeta could use it. I think it was around the time 104 came out.

What's there to explain ? he can use SsjB and an evolved form of that so why wouldn't he be able to use what's below them ?

Now that SsjG Vegeta is a thing in anime form, he'll start making his way into the games so hopefully he gets into XV2 in the next DLC pack.
