I swear, the writers have forgotten some of the story from past events. People keep saying It's just Trunks' anxiety disorder but the Trunks' that left back to destroy the Androids and Cell seemed assured and calm. I doubt Black coming back and destroying everything erased his memories of the time he spent in the past. When Krillin asks for his wish to remove 18's parts, Trunks responds with "What's the big idea, why're you so bent on helping those two anyway." And Krillin gives his honest answer, that he has feelings for her.

Even if it's been a long time, and fought for his world. He should still know she's chill. There should be no reason for him reach out for his sword and attack her. I don't know, that's just how I feel.

Shenlon wasn't able to convert her into a human, as he stated it was beyond his power to do. That's why they settled on removing her bomb, since it was the best option available.

As for why he reacted like that, it makes sense really. He's been on edge for years now due to seeing his world being ravaged yet again, and has just experienced both the death of his mother and the apparent death of Mai, so he's understandably a bit jumpy (hence his initial reaction to seeing Goku). So just imagine him already on edge, and then suddenly seeing #18, half of the duo that resulted in nearly all the Z Senshi (including Gohan) being killed, appear for seemingly no reason, and it makes sense that he'd be shocked a bit.

Maybe because #18 is still a Android/Cyborg/whatever the fuck you wanna call them. The wish only got rid of the bomb, not all her (and #17) robotic parts.

It's been an extremely long time since Trunks saw the main timeline 18 or Krillin's wish, like over a decade for him. His first thought upon seeing either of those androids would be how they killed everyone he loved in his timeline.

He was plenty quick to calm down, he didn't even actually attack before Krillin explained everything. He just cautiously reached for his sword, without drawing it.

It's pretty obvious that Trunks suffers from PTSD and anxiety. Hell, he carries a sword around just to feel at ease. Seeing one of the two people who killed his master and terrorized his world for over 17 years is gonna throw him off, regardless if she's from another timeline or an ally. It's the same face he feared for years. The fact that Trunks didn't obliterate her on the spot shows that he actually sortaa kept his cool.

As everyone has basically said so far, 18 is still an Artificial Human, the wish from Shenlong was to remove the bombs inside her and 17, not make them normal humans. Also if you were Trunks who lived his whole life in hell caused by both 17 & 18, you would do the same after seeing the one who ruined most of your life in front of you alive again.

I've always theorized that the reason turning 17 and 18 into humans was impossible for Shenron is because the wish was nonsensical, they are already humans with cybernetic enhancements, and thus can't be turned into what they already are.
