Is this form of Broly designed by Toriyama?

I don't think any of his forms were aside from base, as that was all that was shown in the ONLY art work that even indicates toriyama had anything to do with him.

However, that's just his super saiyan form, either discolored due to the "color flip" that occurs when some saiyans transform (It happened for Bardock too), or due to being held back by the mind control device. Either way, it's just super saiyan.

The acutal green-haired LSSJ form wasn't desinged by Toriyama either?

Toriyama designed all forms of Broli, but he didn't color them.

wow, my memory was way off... hasn't even been that long since I seen that.
however, it mentions no where about his hair color post transformation aside from the colored image of his LSS form which for some reason shows yellow hair instead of green, which we know to be his actual hair color.

Also, in the bottom right, the "LSS" form does seem to be accurately muscled.

In all honesty, it seems as though they had an idea that they wanted him to flesh out for them, then they added the minor details later.

So I would think it's safe to assume that the colorations were the work of the animation team behind the movie and not Toriyama.

Unless another user around here can pull something up that I've never seen before... which at this rate could happen.
