I guess this is more a question of how anime in general are scored, but I'm primarily interested in Dragonball's music. So, my question is, was it Shunsuke Kikuchi who was responsible for actually placing his music in each episode of the show, or was that the job of other people at Toei?
I have another question that sort of ties in to the main topic, and that is, who or what exactly is 'Monolith'? I've tried to look it up, but I haven't found anything. Was it a group that only made music for Dragonball releases, or something else?
Any help would be much appreciated.

That's hard to answer. Shunsuke Kikuchi did make all the BGM's, but who knows if it was him who puts all the BGM's in every episode scene.
