So I was finally checking out the 'Perfect Cell' Box set, which contains 4 DVDs;
Hunt for 18, Unstoppable, Temptation and Perfection
But on the back, the episode listing and numbering is all fucked up.. very annoying.
Hunt for 18:
Sacrifice #138
Saiyans Emerge #139
Super Vegeta #140
Temptation:
Bow to the Prince #156
Hour of Temptation #157
Krillin's Decision #158
Perfection:
The Last Defence #159
Cell is Complete #160
Vegeta Must Pay #161
then Unstoppable:
Trunks Ascends #147
Saving Throw #148
Ghosts of Tomorrow #149
The Cell Games #150
What the hell happened? how did 'Temptation' and 'Perfection' get so screwed up? Were these DVDs re-issued with correct numbering at any stage? I bought this box set brand new (sealed).

This same thing happened to me a few weeks ago! However, after checking out the Home Video Guide on Kanzenshuu, all was cleared up.
Essentially, the reason for the gap is because they started using the actual episode numbers. Previously, they had been missing episodes because of the edited release of the Saiyan Conflict and the Namek Saga. When Funimation edited these, the episode count was only at 53, which threw off the numbering of their releases afterward. For some reason, in the middle of the Perfect Cell releases, they decided to change the episode numbers so that they accurately matched the originals, but ended up switching back shortly thereafter.

Ahh yes, that is what they must have done by the looks of it, very strange to do it to two volumes, and then revert back to the original numbering. I guess people got confused and thought they were missing out on episodes #141-155

Yeah, two particular volumes ended up with the original Japanese numbering. Scott Morgan can clarify, but I believe that the music producers were ALWAYS given the original Japanese numbering to score to/from (despite the fact that the dub was going with the reduced-count 270-whatever at the time), but I'm not entirely sure how that would have also made its way to the packaging, which was/should have been an entirely separate production area.
