We don't have zero hints or backstory to make an guess. Wait I change that. The only hint we have was that Vegeta re-color when Goku went SSG. I don't really care for that one. Since it showed Vegeta with his current amour with the only difference is he was a tail. It was just a simple re-color + a tail. So we can't say who the first SSG was. We just know that they was some good saiyans and there was some evil saiyans.

I doubt it. If they don't tell the original SSJ(Bardock was the one who started the legend for Freeza)...I doubt they tell the original SSG

It likely happened out of coincidence.

The Saiyans likely did what Goku and company did during the first attempt at SSJG but happened to have the right number of Saiyans and thus instead of pouring their energy into the Saiyan, they inadvertently activated the SSJG ritual.
