I just noticed this while looking at Dragonball Z anime DVDs at my local bookstore. Did the Pioneer staff simply redraw Goku and Vegeta in their base forms for the DVD art cover from the promotional art for Movie 12?

Another possible theory is that at Toei, the art for the first one was done in the appropriate era, then when doing the artwork for Movie 12 that was the one sketched over.
On a similar topic, has there ever been a case of a distributor like Pioneer or FUNi making their own original artwork like that?

Funimation either had a freelancer or in-house artist draw the cover art for Movies 6, 7, and 8. I forget the artist's name, or if they were even credited.
That same artist also drew the cover art for the first Budokai game, and possibly others.
This is the episode of when Gokuh enrages himself after Freezer talk shit about Kuririn

I always loved the box art for the Pioneer releases. I can't imagine it would've been that expensive for FUNi to invest some resources into something like that. Instead, we got awful screenshots.

Better both of them than outright lying covers. Look at this cover for a video my dad bought back in 1992:
One of the four peope on here does not appear at all or is even mentioned, thus confusing my dad when we watched the video. Hence why I'd rather have screenshots than lies.

Yep. My dad bout it thinking it was going to be the good 60s one that he watched as a kid. The back also mentioned Torch being on the team, so it was also lying.

I never watched the 60's Fantastic Four cartoon. I've only watched the 90's and 00's cartoons. The former tends to be criticized a lot, but I always liked the voices for Reed, Sue, Johnny, Ben and Doom.

I don't think so, Goku on the bottom looks just like from the end of the second DB opening:
It appears that it was flipped, traced over, and the Kame symbol moved (but not resized).

If they did they did a pretty thorough job, not only changing the colour of Goku's gi but also adding the kanji and belt too as well as adding Goku's loose hair strands in base. Top job.

It's whatever art they were given, cleaned up. The Pioneer discs feels like they exhausted every piece of art they got.
That Dragonball DVD may have sourced the cover from screenshots but they were all redrawn and look fine. Sometimes you don't get tons of art for shows. For our Lupin series 2 covers, we redrew iconic shots from the intro to build it.

People seem to love those covers and they weren't lazy. They were a lot of work getting an artist (or me) to draw more detailed faces and stuff.
The mysterious adventure cover is a collage, but a good one! They did a good job redoing Goku and putting together a good cover for what they had.
