What I remember most vividly is Missingno and how it could glitch your game forever if you tried to go after it (in the original red/blue games).
I think that rumor turned out to be true though. Some people said Missingno was originally supposed to be data for Mew.
Oh! And for on-topic; my biggest mistakes regarding DB were thinking Yamcha was Goku's father, and that Freeza was a woman. (Some kind of evil queen of the universe).

The first thing of DragonBall I ever saw was one of the segments where Goku is training on his way to Planet Namek. So I assumed Goku was a spaceship captain.

Missingno was a real one. If I remember right it was caused by an oversight in the way the Pokemon were numbered, resulting in it being the default number 000, or something like that. No, the Pokemon numbering used the hexidecimal system to determine identities, and since there were only 151 of them, if you ended up in a battle with one of the 105 hex numbers not used for an existing pokemon used to determine who you face, you end up facing a Missingno glitch pokemon. Of course, this isn't supposed to happen during normal play, the glitch basically involves watching the one computer-controlled battle in the game (an old man demonstrating how to catch pokemon), which temporarily overwrites the "pokemon in this area" data with the name you gave your character so they can use "OLD MAN" as a trainer name for that one battle without completely overwriting your name, then going straight to the one area in the game that has wild pokemon battles with no destignated pokemon to it, a small strip of water right off the coast of Cinnabar Island. Getting into a battle there wil simply make the game use the still-existng data from the previous area (which means it can also be used to face wild Safari Zone pokemon in standard battles), and since that is currently holding the data for your name instead of actual pokemon list data, you end up with identification numbers that don't always correspond to an actual pokemon. It also has to do with what characters you have in your name. My biggest "Mess up" would probably be thinking that Majin Boo and Kaio were the same person, at this point I had played more of the video games than I had actually seen of the series.

Ah yeah, my biggest messup was definitly back when I first played Butoden 2 with absolutely no clue as to the plot of the Cell saga beyond the very bare minimum, and somehow got into my head that the badass green villain guy was this "Mr. Satan" character I had heard of, while mistaking the actual Mr. Satan for Chapa-ou.
