Japanese Wikipedia says he came up with the name "Dragon Ball Kai", though it doesn't cite a source. And that's basically it. The booklets that come with the Kai home releases in Japan go into some detail explaining the production of the show, but never mention any involvement from Toriyama at any stage, as far as I know. The "Toriyama edition" or whatever phrase it was they used to promote Kai early on seems to have no meaning other than that the show was edited to be closer to the manga.
Added: For further proof, the episode credits for Kai only list Toriyama as "original author" (gensaku). This is the credit he's given in all DB anime, as the creator of the original manga they are all ultimately based on. It is, in fact, precisely the same credit he got for Z, as well as for the original DB anime, all the movies, and even GT. The only anime with a different credit for Toriyama is the Jump Super Anime Tour special, where in addition to being listed as "original author" he's also credited for the "original draft" (gen'an) while Takao Koyama wrote the final script. The credits for Kai list those in charge of directing, series organization, scenario writing, and all the other vital jobs, but Toriyama is never listed in any of these credits. Unless there's some vast conspiracy to massively screw Toriyama over in spite of the huge advertising potential such direct involvement from him would generate, we can safely conclude that he had no role in the production of Kai beyond creating the manga.

I definitely recall hearing that Toriyama came up with the "Kai" addition to the name, but I have no idea how true that is.

Why would he have any involvement? All they have to do is go through the manga and cut out whatever they think they can get away with cutting out of the anime to make it closer to the manga. Why would they need him on board to do anything?

I think, honestly, all (at least for Kai's release) they would need his seal of approval...right? I mean, he could be involved in saying, "Yeah, take this, this and this out."
