The first box included both TV specials of Z. The GT special was in the GT Dragon Box.

Those aren't covers, they're actually movie posters. Most of the movies have two or three different posters that were used to advertise the movie, one of which is used as the actual cover. A good example is DBZ Movie 2, which has three. Unless you live in Japan, you probably have only seen two of the three as one of them has never really been used as anything other than a promotional poster in Japan. Real shame too.
