I finally figured out what was bothering me about Emperor Pilaf's wish, apart from the usual plot hole complaints:
"I wish you were a little kid again"...
When a literal genie takes what you say after "I wish", don't they usually use your exact words, and act as if you were talking to them even when you are talking to someone else?
Therefore, shouldn't the dragon have interpreted "you" to mean himself?
You can't argue that the dragon was smart enough to understand that "you" meant Goku, because he obviously took everything that was being said as a literal wish and wasn't smart enough to make the distinction between an intentional wish and an exasperated lament. If the dragon was able to discern the intended recipient of "I wish you were a little kid again", then he should have realized that he wasn't intentionally making a wish.

None of the the dragon do the literal wishes or the Monkey's Paw ones that fuck you over like other genies in works of fiction often do. They understand what you want without worrying too much how you phrase the wish, Ultimate Shenlong is just a jackass and didn't care that Pilaf's wish wasn't an intentional one. He just wanted to go back home.

Not sure; but that would certainly explain it.
I am watching the dub for a series of reviews...requires lots of Tsing Tao
