Movie wise, I would have to say Goku took the worst beating. I mean, have you ever seen the battle against Broli? He jumped over five times on Goku's abdomen as well as proceeding to mercifully punch his it through a large boulder. Out of that entire film, those scenes were the most brutal since you could actually feel the pain of the character. However, series wise - I would have to say Vegeta took the most beating out of everyone in the series. He would make an epic entrance only to be badly torn to shreds by the villain. This mostly implies to his battle against Kid Boo.

I think Goku takes the worst beatings in the series. It's a toss up between his fight with Piccolo at the 23rd Budokai and his fight against Vegeta. Both times he is immobilized because his limbs are broken. Piccolo breaking his bones makes me cringe. At least Yamcha passed out, Goku felt every second of it.

Majin Vegeta against Majin Buu was a pretty good beat-down. SSJ2 Gohan making Cell throw up 18 was brutal, too. Nail's fight against Freeza also gets an honorable mention.

Everything. You think her fight would have the same reactions and be mentioned so many times in this topic if she wasn't a woman? That's pretty much most of the reason why people think its so brutal. There are lots of fights in Dragon Ball where the fighter gets way worse injuries than Videl got.
