Ultimate Gohan is his official name from the video games, yes. The manga makes no difference. Just as Goku surviving the Water is just called Goku or Gohan and Kuririn with the Elder Namekian Power Up are just called Gohan and Kuririn. So, yes. If there is a name that is not from the fans, it is "Ultimate Gohan".

I think so, but it's bled into a LOT of other products since then. If I remember correctly, the Daizenshu just call him what Elder Kaioshin said he would become in the manga: Saikyo no Senshi - Strongest Warrior.

I don't think it's ever used in the series proper, no, but that's underselling it a bit. It's on at least a half dozen figures, I think a couple of trading cards and Gohan's bio in the respective Season Set booklet. There could be other uses as well, but that's all I remember.

Gohan's "ultimate form" is never actually named in the series, nor is it in the guidebooks to my knowledge. Instead, it's described as a power up that will make Gohan the "mightiest of warriors" (saikyo no senshi). It's similar to, though not exactly the same as, Goku describing Super Saiyan God Super Saiyan as "Super Saiyan with the power of Super Saiyan God" without actually giving the form a proper name.

Gohan: “Ho-how can I turn into this mightiest of warriors!?”
Elder Kaioshin: “You transform into that Super whatever-it-is a lot, right? You’ll be fine if you just get the gist of that, and throw in a kiai.”
Gohan: “The gist of Super Saiyan…I-I got it…!”

The Daizenshuu uses the saikyo no senshi expression to at least call it something, but I would say that's still less of an official title than it is a description.

Probably due to fans wanting/needing something to call this new version of Gohan, even though it's still base Gohan in some sense, fans took to calling him either "Mystic Gohan", referring to the magical ritual he underwent, or "Ultimate Gohan", where "ultimate" is a derivative of "mightiest of warriors" and/or just "mightiest".

Buu's Fury also used the DBZ Movie 12 fansub line, "I am the instrument of your defeat!"... so... I dunno what I'm really saying there, but that game was a labor-of-love mish-mash of anything and everything they wanted to do or say.

Anyway, my main point was that "Mystic Gohan" is never actually spoken aloud in FUNimation's English dub production, and was only later appropriated as an "official" term with/via ancillary products. Sounds like it showed up a lot more than I thought, but still. Yep!

I use mystic Gohan. It fits because that isn't gohan's strongest form so it isn't the Ultimate Gohan,and his power up is by a God so it's a mystic power up. He's been effected by the magic as he also has a advantage added to his body. No transforming to get to 100%,transforming takes up energy and strains the body so there's also that to the power increase.
