It's common manners not to call people you aren't close friends with by their first name everywhere I've ever lived. That's where the term "first name basis" comes from. What is the problem? You are clearly adjusted to different cultures and ways to show manners.
As long as we know that someone doesn't mean to show any disrespect (which is doubtful that one would do if you join a forum regarding one of the Author's works which you love), why make such of a big deal out of it?
I don't see why people always are trying to force their views to eachother.
Maybe it's common in the States to use the last name, and maybe in Piccolo Damaioh's city alone, it's not!
Words and names are just what they are: words and names. Proper etiquette is to be able to have a discussion and still try to understand that other people's reality isn't the same as yours, and not getting all hung up on if someone uses a person's first or last name.

There is only a few places were you would even need to address someone by calling them Mr.(lastname)

I can see why some people were taken aback by it, seeing as it's common to refer to someone who you are not personally familiar with by their surname, but now I'm hearing people saying it's bad manners. No, it's not "bad manners". It's just that it's seen as strange to refer to someone you're not familiar with by their first name. By using "Akira", it seems as if you know them personally. Like, during commentary for sports such as football (or soccer in America), they don't refer to David Beckham as "David"--primarily because it's a matter of specification, as there are other players with the first name of David, but also because they're not personally familiar with them--but they refer to him as "Beckham". Going off the "familar" track, it's not just a matter of familiarity, but also "specification", because everyone knows who "Beckham" is, and not who some random "David" is.
Frankly, I don't even care whether people use "Akira" or not, just that it would look weird to me. But I would ignore it, like a mature human being should.
