Was it created by Akira Toriyama? I don't remember it being in the Manga.

No it isn't in the manga anywhere, and as far as I know he didn't design King Vegeta's outfit. Could be wrong, though.

He only designed King Vegeta's face, so odds are he had nothing to do with the crest.
