The whole cover (and the other singles) is incredibly poor (Goku doesn't even look right).

I don't know, the first two seemed pretty good to me (even though you can tell most of the movies in it).

I think I remember seeing a busted up one during a yard sale once (didn't buy it though).

The sequel was originally announced for later the same year as the first Transformation. We have literally no information on it right now. I think either Webfoot is gearing up for something huge (unlikely), or it's vaporware (most likely, but unfortunate).
